Arizona Home Based Services (HomeBase) is contracted with the Arizona Department of Economic Security and the Division Developmental Disabilities (DDD) providing home and community based services (HCBS) throughout the valley.
HomeBases’ mission is to serve as a bridge between our members and the world surrounding them. We aim to enable the members, one step at a time, to grow and function more fully as beneficial members of society, thus creating a more valuable community for us all.
We understand that finding the right provider is important. We will take the time to get know you and your family to find the perfect fit.
With more than 15 years of experience in the special needs community, we have seen the importance of quality care. We started this agency in 2016 with one goal in mind; to provide the highest level of care to as many (dis)abled people as possible! We love what we do!
Services
Services are offered in the following Cities:
Chandler, Gilbert, Mesa, Queen Creek, San Tan Valley, Tempe, Scottsdale, Tucson, Maricopa, Casa Grande, Phoenix, & Show Low.
HABILITATION
Habilitation (HAH) is self-help skills taught in steps to develop more independence and confidence. Habilitation includes a variety of methods such as: special development skills, behavior intervention, sensory and motor development skills. They are all designed to maximize the functioning of persons with developmental disabilities.
RESPITE
Respite service (RSP) provides short-term care and supervision in accordance with the members Person Centered Service Plan (PCSP). The goal of the service is to provide a “break” for the caregiver. This service can be provided in the person's home or the community.
ATTENDANT CARE
Attendant Care service (ATC), or Total Care, provides necessary support in order for the person to remain in his/her own home and/or participate in work or community activities. The goals of the service include assisting the member to have a safe and clean home, stay healthy and have good meals.
